Please call us at 360-679-8946 to make an appointment at either our Oak Harbor or Greenbank office. If we are busy with another patient, please leave a message on our voice mail. We return all calls as soon as we can.

Patient Intake Forms

If you are an adult patient, you can download a PDF version of our patient intake form. We also have a patient intake form for minors.

It is very important that you have your patient intake forms filled out before seeing the doctor. If you have not downloaded one and filled it in before arriving at our offices, please arrive at least half an hour before the time of your appointment in order to give yourself time to answer all the questions.

It is best to download the appropriate form from this page, give yourself time to answer the questions and arrive at our office with it already completed. We understand that we ask for a lot of information and that you may not have all the requested information readily available. However, please answer as best you can because naturopathic medicine relies on your physician understanding as much as possible about your condition in order to provide you with the best care. Of course, if you do not understand a question, please feel free to ask what it means.

Records Release Forms

If you need to obtain your records from a previous healthcare provider, you can download a PDF version of our records release form. This form should be completed, signed and dated, and sent to your previous healthcare provider. If you need help filling it out, please ask.
